How to fill out the title page of the income and expenses book
Ledger income and expenditures - a report document that is required to conduct individual entrepreneurs and enterprises using a simplified taxation system, including those who do not actually carry out activities. Title sheet must be filled in anyway. To do this, the entrepreneur or representative of the company is required to enter the corresponding values in the columns intended for them.
You will need
- - a paper or electronic form of the income and expenditure accounting book;
- - requisites of individual entrepreneurs or firms;
- - computer or pen.
Instructions

Specify in the field located at the top of the title page, the year for which the book of income and expenditure is maintained.

Type in the string for the nameentrepreneur or the name of the taxpayer organization, your full name, first name and patronymic completely, if you are a PI, or the full name of the firm with an indication of its organizational and legal form. For example, Ivanov Ivan Ivanovich in the first case or Limited Liability Company "Horns and Hoofs" in the second.

In the lines below, specify the TIN andthe presence of a checkpoint. The upper line is for the TIN and CAT of the firm, the lower line is for the TIN of the entrepreneur. Keep out of the ordinary for your case. So, if you are a PI, you do not need to write anything in the line for the TIN and KPP. The same goes for the line for the TIN entrepreneur in the case of the firm.

Write in the next your object of taxation: income or income, reduced by the amount of expenses.

Specify in the line for the unit of measure of income "rub.", That is, rubles.

Enter the address of the registration in the appropriate field if you are an individual entrepreneur. If you fill the book of incomes and expenses of the firm, enter in this line its legal address.

Reflect in the next line your settlement accounts of a legal entity or an entrepreneur: a twenty-figure account number and the name of the bank in which it is open. If there are several accounts, please specify all.

Enter the notification number in the appropriate fields about the possibility of applying the simplified taxation system and the date of its issue.

Do not fill the bottom line of the title page.sheet. It is designed to be signed by a tax inspector who will certify your book of incomes and expenses (legally the paper version is verified after the completion of the title page, before the other entry is included in the other sections, the electronic printout - at the end of the calendar year) and its decoding.
