Attach to the application files with the scanned originals of the passport of one of the parents, the birth certificate of the child.

If you are satisfied, you can start a medical commission. To do this, go to the children's polyclinic at your place of residence. When a child's medical card is passedthe commission will be in your hands, you can go to the preschool authorities to conclude an agreement. Bring your passport and birth certificate. If necessary, take with you the original certificate of temporary registration at the place of residence.
