First of all get from the employee a sheetincapacity for work. The document must be issued in a medical institution. Check that it is filled correctly. In the line the place of work and position should be written reliable information, that is, the name of the organization according to the constituent documents, and the post according to the staffing table. The reason for incapacity for work should also be indicated.

If you make out a sick leave sheet forpregnancy and childbirth, check the date of maternity leave. To do this, from the estimated date of delivery, which is indicated in the hospital sheet, take 70 days for singleton pregnancy and 84 for multiple pregnancy.

If there are corrections in the disability sheet, they must be certified with the signature of the doctor and the seal of the medical institution. Not allowed more than two corrections.

But this document is needed only if the hospital is given for pregnancy and childbirth, the basis for its compilation will be as follows: "on granting maternity leave and payment of benefits." In the hospital sheet, fill in a special section. Here, specify the data of the employee (name, taxpayer identification number, SNILS), insurance length of service, period of incapacity for work. Just below, enter the amount of the average daily earnings, the total amount of the allowance. Indicate how much money was paid at the expense of the employer, and how much at the expense of the FSS. Sign the document.
